List of Required and Supplemental Documents

Please use the below checklist to assemble the required documents in the following order and bring to the immigrant visa interview:

  • U.S. tax documents (Form 1040) or other financial evidence (from the most recent tax year)
  • Proof of U.S. petitioner’s citizenship or LPR status (such as a photocopy of a U.S. passport or naturalization certificate)
  • Proof of U.S. petitioner’s domicile in the United States (Important: If the U.S. petitioner does not currently live in the United States, please ensure you provide sufficient evidence of their domicile in the U.S.)
  • Offer of Employment to Alien (if any)
  • Panel Physician Medical Report  (Important: Hospitals will require two weeks to complete your examination. If your medical examination will not be complete by the time of your interview, please contact us to reschedule your appointment.  Otherwise, your visa cannot be approved.)

**Documents not in Chinese or English must be translated into English by a certified translator and sworn to before a Notary Public.**
