Nonimmigrant Visa Application Forms
DS-160:
Each nonimmigrant visa applicant must fill out an online DS-160. (Including K fiancé/fiancée visa applicants. Please note that K visas are processed by the Immigrant Visa Unit.)
IMPORTANT NOTICE: You must fill out your DS-160 completely and accurately. Applicants are strongly encouraged to prepare relevant documents in advance when preparing to fill DS-160. Questions you may be asked in DS-160 include:
Personal Information:
- Date and place of birth
- Home address
- Phone numbers
- Passport number, issuance and expiration dates, place of issuance
- Salary in local currency
- Spouse and Parents’ names and dates of birth
- Present and past education: including names and addresses of schools attended, starting from middle school, and the degree earned or subject studied
- Work experience, including address of company/organization and a description of your general work responsibilities
- Other relatives in the U.S.
Purpose and History of Travel:
- Itinerary, including (if already scheduled) flight information, intended travel dates, address in the U.S. and purpose of travel
- U.S. contact’s name, address and contact information
- Previous U.S. travels, including dates and duration of stay
- Prior U.S. Visa issuances, including the date of issuance and visa number
- Countries visited in the last five years
- If applicable, questions about prior military service
Please note that based upon your situation and the type of visa required for your purpose of travel, the DS-160 may ask you to answer additional questions required by U.S. law and regulations.
Important Steps:
- Save your application regularly to avoid “time out” issues
- Print out a confirmation page with a bar code after completing the DS-160 form
- Attach a 2”x2” square inch photo on the lower left-hand corner of your DS-160 confirmation page
Applicants with inaccurate or incomplete information on the DS-160 at the time of their interview will be asked to correct and complete the form online and return to AIT for an interview within 5 business days. Review the Frequently Asked Questions or the Department of State website for instructions about the DS-160 form.

DS-156E: Nonimmigrant Treaty Trader / Investor Application (PDF file)
Treaty trader (E-1) or Treaty investor (E-2) applicants must also complete this form. Print and complete this form, then bring it along with the DS-160 confirmation page to the interview.
NOTE: When printing out the DS-160 confirmation page and/or the DS-156E form, please use standard blank white paper. AIT will not accept forms printed on any of the following: colored paper, paper with watermarks, "recycled" paper (with printing on the back side), and thermal transfer paper.
